Student Health Information and Requirements for 2016-17

Dear Parents and Guardians,  

All Gonzaga students must have a completed health record on file with Gonzaga’s School Nurse, before the start of each school year.  

We kindly ask all health records are up to date by AUGUST 1st, 2016.  

A completed health record at Gonzaga includes the following:

  1. Physical Exam & Immunization Record

  2. Careflow Electronic Health Record→ Online Access June 1, 2016

  3. Emergency Medical Treatment FormFor new freshmen or transfer families only  

  1.       Physical Exam & Immunization Record 

The District of Columbia requires all students to have an annual Physical Exam and Immunization record on file.  The physical forms (Health Certificate) link includes 3 documents, the exam by a health care provider, Immunization record and Medication permission form. 

The Physical exam must be current within one year, (365 days) to the day of the last submitted physical for participation in Athletics, PE and extracurricular activities.           

Students without current physicals may not participate in tryouts, athletics or school activities. We strongly recommend that your son have his annual physical during the summer months, this will prevent him missing classes and/ or athletic practices.  We understand that insurance and birth dates may not coincide with summer months, please arrange appointments as soon as possible.  Our mission is to provide the optimal health care and safety for your sons. 

2.         Careflow: Electronic Health Record 

Gonzaga has partnered with CareFlow Electronic Student Health Records to coordinate health and emergency information. All parents/ guardians must complete the on-line Careflow parent portal health history and emergency fields to ensure the best care at school and preparation in the event of emergency or the need for outside medical care. As noted, parents may log in on June 11th as records will updated after graduation. 

3.         Emergency Medical Treatment Form → For new and transfer families only 


Welcome new families!  This form is completed before your son’s first year at Gonzaga and kept on file in the Office of the School Nurse in the event of an emergency.  If your son is attending our freshmen transition program we ask he arrive on June 27 with this form in hand.

Health Office News and Notes

Please report any contagious conditions or health concerns to the School Nurse. Please refer to the Student Handbook for the health absence reentry policy for short term and extended absences.

Concussions and Injuries 

Gonzaga’ students are very active and experience various injuries. The Athletic Trainers and School Nurse work together to support your son’s recovery and return to school. Please contact us with any injury involving your son’s health and school attendance.

Head Impacts and Concussion numbers continue to rise in student athletes across the nation. To prevent long term effects, our Concussion team recommends any Gonzaga student with an injury to the head to be evaluated by the school nurse, athletic trainers or a medical practitioner.

The Athletic Trainers, Counselors and School Nurse work together to identify, educate and create a strong recovery plan. Recommendations and assessments will be made for the students return to school, monitoring physical symptoms, academic support, and a gradual return to play.
